Loss Notification
If you've experienced a loss, please contact our Care Team by submitting a Loss Notification form below. Once submitted, we'll email you a secure link with the checklist of required documents and the form to request funding.
Household Update
Household updates can be made at any time. Just submit the Household update form and your records will be updated automatically. Only 10 household members allowed.

1. How do I report a loss?
Notify our Care Team within 14 days of a loved one’s passing using the Report a Loss form in your portal.2. How do I request support funds?
Use the Request Support form in your portal to access burial or cremation benefits. Funds are released quickly once approved.3. Who can use my membership benefits?Only household members listed on your plan are covered. Each individual may only appear on one membership.4. Can I update my household?
Yes. You can add or remove members anytime using the Household Update form in your portal.5. Is there a waiting period before benefits can be used?Yes. A standard waiting period of 12 months applies before new members can access full benefits. This will be outlined in your plan details.6. Are we a funeral home?No. We are not a funeral home or provider. We offer financial support to assist with burial and cremation expenses at the funeral home of your choice.7. Can funds be paid directly to a funeral home?Yes. At your request, we can release funds directly to your chosen funeral home or provider.8. What happens if I miss my membership renewal?Benefits pause until your membership is active again. To avoid gaps in coverage, renew or set up an installment plan before your expiration date.9. Can multiple claims be made under one membership?Yes — but only up to 1 per year per household for household members listed on your plan, and each loss must be reported within 14 days.10. What documentation is needed to request funds?You’ll need a death certificate of the deceased, or proof of relationship, drivers license, marriage license (if you are the spouse), etc., and a completed Funding Request form.11. Are refunds available if benefits are not used?No. Membership fees are non-refundable, as they secure access to pooled funds and program resources.12. How quickly are funds released?Once approved and verified, funds are generally released within 3–5 business days.13. Can I have more than one Legacy Contact?No. Only one Legacy Contact may be listed per membership, but you can change them anytime in your portal.

About Kay

April 3, 1956 - February 22, 2021
Resilience
Born and raised in the Historic Westide of Las Vegas, Nevada - a community at the time marked by segregation, limited infrastruture, and systemic barriers - Kay and her siblings were raised by their maternal grandmother after the loss of their mother when Kay was just eight years old. She came of age during the height of the Civil Rights Movement, as Las Vegas witnessed the historic Moulin Rouge Agreement - an unprecedented step toward desegregation in casinos - and as Nevada faced rising calls for equity and justice. In the midst of these sweeping societal changes, Kay navigated her own personal struggles, including the loss of her younger sister, her aunts, uncles, and later, her father and grandmother. Life tested her with many hardships, but she carried each challenge with grace and resilience.
Mother & Entrepreneur
As a devoted single mother, she'd work full-time and take on extra gigs to provide for her daughter. She often relied on her entrepreneurial abilities and used her skills by providing hair care and styling services to friends, family, and neighbors. She also cooked and often sold out of her famous tamales and egg rolls, and would crochet blankets, hats, scarves, and outfits upon request, just to make ends meet — always finding a way to keep food on the table and provide what was needed. She never allowed struggle to define her.Determined to create a better life for herself and her daughter, she returned to school - still working full time - and secured work as a CNA, later becoming a Medical Assistant, and Phlebotomist. This decision allowed her to build a home in a better neighborhood where her daughter could thrive.

Service
Even while facing her own health challenges, she poured herself into others. In her life, she fed and cooked for her community, ensured over 80 families had food for the holidays, participated in several donation drives for the unhoused, clothed and poured life into several children, and always had a heart to give. Her life is a testament to service, compassion, and justice. She stood up for those who didn’t have a voice and protected the vulnerable. She believed the future lives in the people closest to us right now — and that the future is always worth protecting.
Her Transition
After battling cancer courageously for five years. She passed peacefully while on in-home hospice care at her daughter's home, with her daughter by her side until her final breath. She leaves behind her daughter, beloved grandchildren, and a host of loved ones, who carry her light forward.

Legacy
Kay was a leader, visionary, and a woman of impact — someone who stood for purpose, advocated for the vulnerable, and believed in creating spaces for others to be heard, seen, and not forgotten. Her values were clear: fairness, community, compassion, justice, and equity. Principles that have now become the foundation of The Kay Trust - a namesake social enterprise - in her name.Her story does not end with her passing. Instead, it lives on as the inspiration behind the programs of The Kay Trust — from financial assistance and wellness programs, to community care and legacy preservation. Not only do these programs reflect her deepest beliefs, but they also embody a daughter’s promise to uphold them.
